Uterine artery embolization (UAE) is a procedure that can significantly reduce the painful symptoms caused by adenomyosis safely and effectively without surgery.
A 2015 study reported that around 1 in 125 women in the United States are known to have adenomyosis. This number is probably actually much higher because many women go undiagnosed or do not seek treatment. Adenomyosis occurs when the tissue that is usually located in the endometrium begins to grow into the muscular wall of the uterus and starts to cause heavy periods, pelvic pain, cramping, and abdominal pain.

Uterine artery embolization is a procedure that is done by an interventional radiologist and is not performed by an OB/GYN. This procedure requires a specialist with extensive training, like Dr. John Lipman at the Atlanta Fibroid Center.

After you undergo a pelvic MRI, you will begin your treatment for adenomyosis with a consultation with Dr. John Lipman. During your 45-minute appointment, Dr. Lipman will review your MRI images, discuss your symptoms, explain your treatment options, and explain the embolization procedure. Most women who are candidates for hysterectomy surgery are candidates for UAE.
Step-by-Step Overview
Sedation and Numbing
You are given a light sedation medication so you feel relaxed and a local numbing agent is applied to the area where the catheter (thin tube) will be inserted. This type of sedation is much gentler on the body and is not likely to cause the side effects associated with general anesthesia.
Real-Time Visualization
Advanced Angiographic imaging equipment with a monitor allows Dr. Lipman to visualize what he is doing so he can navigate the UAE procedure with precision.
Artery Access
A tiny skin nick is made near the top of the thigh to access the femoral artery.
Blood Vessel Mapping
A special contrast dye is injected to highlight and map out the intricate blood vessels around the uterus so the doctor can identify those that need to be closed off.
Inserting the Catheter
The interventional radiologist will insert a very thin catheter through the tiny incision in the groin and into the uterine artery with the help of an Angiographic.
Embolization
Minuscule, ball-shaped particles are injected through the catheter into the vessels that are feeding the adenomyosis tissue to block the blood flow, which causes this tissue to start to shrink and eventually die.
Completing UAE
Once all the identified areas have been treated, the embolization is complete, and the catheter is removed. Patients leave with only a simple band-aid applied over the small skin nick and no stitches are needed.
Recovery
Patients are observed for a few hours and then discharged the same day to recover at home. The first few days can be uncomfortable or include painful cramping as the treated adenomyosis tissue shrinks. Your doctor will provide you with pain medication. In a week your recovery should be almost complete.

Adenomyosis
Yes, uterine artery embolization (UAE) and uterine fibroid embolization (UFE) are almost identical, and during the procedure, small particles are injected into the uterine artery to block blood flow to the areas that contain the affected tissue or uterine fibroids.
The very slight difference in these procedures is that when performed to treat adenomyosis, the size of the particles that are injected is much smaller compared to the larger particles used when treating fibroids.
